Policy Amendment Request Form

To request for Top up, kindly attach the following:

Duly accomplished and signed Top-up form by the policyowner.
Clear copy of one (1) valid government-issued ID or two (2) valid non-government IDs of policyowner/ authorized representative or sole proprietor, and if any, of the irrevocable beneficiary.
Mode of Release form (if PRULink Cash Flow Fund is chosen).
Proof of payment of the top-up amount, which you can pay through the available payment facilities . Here are the minimum top- up amount per plan, rider and mode of payment:
- For PRULink and PRUHealth Prime plans - PHP20,000 or USD500
- For PRUMillionaire - PHP40,000 or USD1,000
- For PRUMax Invest Variable Life Rider - PHP5,000
- For PRULife Your Term Variable Life Rider: PHP5,000 (one-off or annual), P3,000 (semi-annual), PHP1,500 (quarterly), or PHP 500 (monthly).
